Annual, Post Assembly and Part Replacement inspections NEED to be performed by a qualified person.

This form is to notify management that a LOTO needs to take place. Please follow proper procedures found in the procedures tab to ensure devices and equipment are properly locked out and tagged out. 

Use this form to upload any forms you need to have on file with SCS. Ex. Shot Records, Training certifications, Respiratory fit testing, ETC

Form can only be filled out by a Competent Operator 

This will be filled out when doing any work that produces a spark, flame, or heat.
